Do you have a Maine drivers license or state ID?*
Maine law requires that a medical provider physically see the patient's Maine Drivers License or State I.D Card before approval. If no proof, certification will NOT be granted.
The Relief Clinic and its providers has no financial interest in the establishment that we send you to. You can go to any dispensary of your choice.
